Our services are available to all community members living in Seward and the surrounding areas who

• have a life-threatening or terminal illness

• have been given a life expectancy of six months or less

• have continuing medical care from their primary health care provider

Referrals can be made by friends or family of a person in need, by health care providers or by the individual. If you or someone you know would benefit from hospice services, please fill out a referral form found at the link below and return it to us by mail, email or in person at the office. You can also call us at 907 224 3051 to make a referral over the phone or make an appointment to talk about the process.
